Please call or text with any questions or if you are unsure whether a condition applies to you.
Some conditions do not automatically prevent treatment but may require medical clearance, additional evaluation, a modified treatment plan, or postponement of treatment. Final treatment eligibility is determined by the treating provider based on your medical history, medications, previous treatments, current condition, and overall safety.
Please notify our office as soon as possible if any item listed applies to you. This allows our team to determine whether your appointment can safely proceed or if additional clearance or scheduling adjustments are needed.
Cancellation & Rescheduling Policy:
If an appointment must be cancelled or rescheduled due to treatment ineligibility, failure to disclose a relevant medical condition, or information provided prior to treatment that prevents the procedure from safely proceeding, the $200 late cancellation/rescheduling fee will still apply when the change occurs within 24 hours of the appointment time. -
